Enrollment Forms

Enrollment Form- Parents/guardians and Students must complete and sign an enrollment form for each student.  Students will not have access to the school computers or their school email account until the school has received the student's enrollment form
(Note:  Student signature is required on the form.) 

Formulario de matriculación de estudiantes - Los padres / tutores deben completar y firmar un formulario de matriculación para cada estudiante. Los estudiantes no tendrán acceso a las computadoras de la escuela o su cuenta de correo electrónico de la escuela hasta que la escuela haya recibido el formulario de inscripción del estudiante. (Nota: Se requiere la firma del estudiante en el formulario.)

IT'S THE NEW RULE A SHOT before your SENIOR YEAR of SCHOOL Ready for 12th Grade?

Starting fall of 2020, a 2nd dose of the meningitis vaccine is required for all 12th grade students.  Make sure your Senior is ready to start school this Fall.

IT'S THE RULE SHOTS before SCHOOL Ready for 7th Grade?

The TDAP (Tetanus, Diphtheria, and Whopping Cough) and Meningitis vaccines are required for all 7th grade students.  Make sure your child is ready to start school this Fall.
